Comenzó tomando inspiración tanto del Doom Metal (el cual nació del Heavy Metal). Suele abordar temas relacionados con el amor, la pasión, la soledad, la tristeza, la sangre, el existencialismo o la oscuridad, sumado a la voz de una cantante soprano, y toma influencia de la poesía, el vampirismo, la literatura, la filosofía y el arte gótico. Esta música consiste sencillamente en la pesadez del metal (más específicamente Doom Metal) con las atmósferas oscuras del rock gótico y algunas de sus características musicales, en conjunto con otros elementos atmosféricos y musicales propios del Metal Gótico, es decir, elementos originales creados a través de la fusión del Rock Gótico y el Doom Metal. Se puede comenzar a hablar de Metal Gótico a partir de la década de los noventas, pues fue durante esa época en la cual nacieron las primeras bandas y con ello el género musical.
Los pioneros fueron bandas como Paradise Lost y My Dying Bride con algunos de sus trabajos, y más tarde, Tristania, The Sins of Thy Beloved, The Gathering, Theatre of Tragedy, Tiamat, Moonspell, Lacrimas Profundere entre muchos otros (la mayoría bandas de Europa), terminarían de consolidar el género.
